An apartment reservation agreement that they can share?

I am looking for a document that contains wording that itemizes total move in costs, monies paid, includes spaces for utility contact information and account numbers, and specifies that the reservation deposit is refundable in the event the applicant is declined or withdraws the application within 72 hours.

It should also include spaces that defines what the prorated rent for the first partial month is and that it gets paid on the 1st of month 2; while move in costs include the first full month + the deposits - funds paid - any concession offered.
