App for Guest Cards?

Right now, our office uses paper guest cards for prospects. Is there an app that we can use for prospects to fill out their info, rather than using paper cards? I've been looking for one but can't seem to find it. Maybe I'm using the wrong key words to search. Any recommendations would be greatly appreciated!
