The NAA has a request for accommodation form that you can give to them and they have to take it to their doctor for them to fill out. If it’s a certificate they paid for the “doctor” usually won’t fill out that form.
Better yet, you should send it directly to their doctor. That's what our company does. We actually had someone in the past fake that it was from a doctor as well
I agree, we had the same thing happen. We have the prospect or resident fill out the request for accommodation but we mail the medical or mental professionals the form from. It never leaves the office with the prospect or resident
No. My attorney advised there is no such thing as an emotional support animal "certificate". These are all the bogus online scams where people purchase them. Send to your attorney!
This is not acceptable. I used to use the HUD form for reasonable accommodation, which has to be signed by the health care professional. Now it is a bit easier to accept the same forms that TSA and Airlines require. No certificate, but a letter from the licensed psychologist noting the need.
Your mental/emotional disability must meet the requirements of the Diagnostic and Statistical Manual of Mental Disorders – Fourth Edition (DSM-IV)
The signing mental health professional must give proof (ie license number) and the jurisdiction and date of that issued license.
Information must be on therapist’s own letterhead and dated no later than one year ago. You can require this be updated annually upon renewal. You may ask an attorney if you may deny:Any exotic animal claimed to be ESA, such as:
Insects
Reptiles
Snakes
Sugar Gliders
Rabbits/Rodents
Hedgehogs
lol. Is your service animal a Cheetah? or maybe a Skunk? How about flesh eating bacteria? Comforting right? No animal too exotic. Guaranteed approval. Please let your residents know that I can provide them with bogus documents starting at $20.00. Have them DM me.
No. Those documentations are available online to anyone. It's actually no more legitimate than going into a word document and making one yourself... The tenant must obtain a letter from their doctor. Specifically, the therapist or psych nurse etc that they are in the care of
We have a form that we mail directly to their health care provider and include a return envelope for them to send back to us directly after it's filled out. That way we know it's going to an actual health care provider's address.
Agreed: we accept documentation from a doctor, service care provider, social services worker or case manager. Like others mentioned, ask for that office and send directly if you feel more comfortable that way. Good luck!
